Overview
The Patient Service Center was created to make it easy for you to communicate with your practitioner, request an appointment, and submit health prescription refill requests. This easy to use website enables you to access many services online 24 hours a day, 7 days a week.
If this is the first time you are using this service, you must register before you can proceed. Registration is easy; just click on the “Patient Login” tab at the top of this page and follow the instructions. We will request some basic information about you to verify against our records. You will be asked to create a username and password to quickly access these online services in the future. If you are already registered, just sign in using your user name and password on the right side of this page.
Once you are signed in, you may access Patient Services, Clinical Forms, and Resources by clicking on the desired tab on the left side of this page.
These services are designed to enhance the way you communicate with your clinician and manage your healthcare. Studies have shown that patients are likely to be healthier when they take an active role in their care. Through these features, we hope to provide you the highest level of service and give you another way to take on an active role in maintaining your own health.
